Adult Team Fee $150.00

 

Participation in the OFCD 35th Adult Team Tournament is for Deaf and Hard of Hearing individuals and their primary language is ASL. 18 years of age and older are eligible to participate in this tournament.  Hearing and non-signers are welcome to participate in the 35th Anniversary events but not the Fishing Tournament.

And whereas participation in each of the tournaments/activities is voluntary and the OFCD requires this release be executed in order to take part in the tournaments:

 

Now therefore the undersigned angler herby agrees to abide by and follow all rules and regulations of the tournaments and agrees to assume all risk and liability associated with participation in this tournament and:

 

Now therefore the undersigned non anglers agrees to abide and follow all rules of the Lower Beverley Lake Park location and agrees to assume all risks and liability associated with participation in activities.

 

The undersigned anglers and non anglers further hereby does release, remise, and forever discharge the OFCD, its officers, directors, and committee from any actions, causes of action, debts, liabilities, claims, demands and complaints of any kind relating to the tournament and participation therein.

 

The undersigned further acknowledges that the OFCD reserves the right to reject any application for entry into the tournament at their discretion.

 

Also, I herby give OFCD permission to use my name and photographs/video images taken during the event to be used for promotional materials or publications at OFCD/OFCD 35th website, Facebook and Instagram.

 

I have read and understood the waiver and agree to follow the rules and policies.
